Waiver / Terms & Conditions
I, the undersigned, or parent/guardian of the Student, do hereby consent to my/his/her participation in Kevin Chin Golf programs (each a “Program(s)”) operated by Kevin Chin Golf INC (the “Company”).
I acknowledge that the Program requires that the Student participate in strenuous athletic training and that the Program will require that my Students travel in golf cars and other motorized vehicles within the Program’s facilities. I acknowledge that participation in a Program may expose the Student to the possibility of injury. I grant the Company the authority to obtain emergency medical treatment as necessary in the event the Student suffers an injury while participating in a Program.
I acknowledge that I have read Kevin’s teaching philosophy for the appropriate category that I am purchasing.
I hereby certify that I know of no medical problems that would increase the risk of illness and injury to my Student as a result of my/his/her participation in a Program. By signing this document, I acknowledge being informed of the risks associate with the Program’s activities. By signing this document, I assume all risk for the health and well-being and hold the Company, as well as its owners, employees, and other authorized agents and independent contractors, harmless therefrom.
Waiver and Release:
I waive any claims, demands, causes of action or any claims for relief whatsoever against, and release the Company, as well as any of its owners, employees, or other authorized agents and contractors from any and all liability, claims and/or causes of action that I may have for injuries or other damages, arising out of participation in a Program.
I accept financial responsibility for any injury that my Student may cause either to his/herself or to any other participant due to his/her negligence while participating in a Program. I agree to indemnify and hold harmless the Company, as well as any of its owners, employees, or other authorized agents and contractors from liability for the injury or other consequences of any injury to any person and damage to any property that may result from the negligence of my Student while participating in a Program.
I hereby grant the Company permission to use a photograph/video image of my Student in any and all publications and media sources of the Company, including the Company’s web site, without payment or any other consideration in perpetuity. The Company will not divulge any names without written permission.
Rain and Cancellation Policy.
I acknowledge that a Program day or class may be canceled at the discretion of the Company due to weather or other conditions. Canceled class credit policies change per Program. I acknowledge that I am aware that the Company does not refund any fees for canceled Program days or classes.
For Private, Semi-Private and Playing Lessons: I acknowledge that there is a 24-hour cancellation policy. Failure to give 24-hour notice will result in the full charge for the session. *NOT including Junior Summer Programs.
For Mentor Groups: I acknowledge that if the mentor group doesn’t meet the required 2 students the class will be half the time for the regular price.
I have carefully read this Agreement and fully understand its contents. I am aware that this is a release and waiver of liability and I sign it knowingly, voluntarily, and of my own free will.
By purchasing, I agree with all of the above.
